To craft Ashes of War artefact, you need to do the following

Confirm your choice

2. Go to any forge, the Ashes of War menu should appear in the Jewelry category

3. 3 gold ingots are needed for each artefacts.

4. Having the right Ashes of War perk for the weapon the artefact you crafted is for

In the example above the Aimed Blow artefact works for several weapon types including swords, so in other words to have this ashes of war artefact working you need

Some artefacts work only for one weapon type, some other for all weapon types.

There are up to 74 different artefacts that can be used with different weapon types, some are offensive but some are defensive as well.

When equipped, the artefact overrides any current stance you are using, in other words when you will press the ashes of war attack button, the same attack animation will always be played.
